He's underweight because he has very low body fat, which isn't great for normal health, but he still has muscles because he's constantly running around and lifting grain sacks, corpses and animal carcasses. Obviously in real life if you don't eat enough eventually your muscles also wither, but I've seen emaciated looking homeless people that still have decent muscle definition.

You also need to give him a sleep right after it, that's a great way to increase weight. Also, if he's already sick, then it's gonna be almost impossible to be anything than underweight

The official guide lists that all meals in the game have specific calorie counts. I think Big Game meat has the most calories. So feed him some Big Game and then get him to sleep. He'll fatten up.

I do feed Arthur and the horse, but I still hoover between underweight and average weight with Arthur. If you want Arthur do get overweight or to stay at perfect weight you'll need to eat a lot of saloon/camp meals, besides other food, it's a chore...

​​​​@@RyanzinBR Just get Big Game Meat, climb or vault something while in the middle of the short eating animation, you gain the weight and the core refills but don't lose the item, it's a really useful glitch and works on tonics as well (if you are dual wielding or holding a two handed gun, Arthur/John needs to put away his weapon before so it's better to do it while empty handed) Alternatively you could just continue getting drunk and passing out because you normally gain +0.75 weight after

@@RyanzinBR Yea, it does take a while though due to Big Game Meat only giving a small bit but I was using it while messing around doing brawls as John in BlackWater and before I knew it I had -3% Damage taken from how many times I fell in water

It would be funny if you could have a really fat Arthur like cj in GTA San Andreas. Instead we only have a bit chubby Arthur. And yes I know it would take some of the seriousness out of it but it still would be a funny feature.

It would be INSANE if your seriousness in interactions would reduce with increasing or decreasing weight. Just like when ur drunk. Like, you rob someone but they dont take you seriously so you would need to draw a gun to actually have an effect on them.

If you are wondering why Arthur has tendency to have a lower weight is that Tuberculosis causing people to lose weight, which is mostly a malign sign of diseases. Rockstar really rocks big time to make a detail like this.

Arthur built like a brick shithouse

I was pretty much always underweight on my first playthrough, used a lot of thyme big game so didn't need to fill my cores that often, this new playthrough I'm using a lot more canned food for quick top ups and cigarettes for dead eye, and it seems to fill my hunger more than just my cores, so I'm currently average, working towards perfect.

Overweight Aurthur isn't even that bad. Better shape than many people to be honest.

Maintaining a perfect weight is a headache, but easy

Those who like thicc arthur are lucky there is a bug that keeps his face big even when his weight falls off. Ive had that happen a couple times where i didnt feed him so long he went underweight but because i reached overweight status earlier in the game his double chin and puffy cheeks stayed until crash landing on guarma where the code forcefully resets arthur to his sick look. But the same does not work the other way around saddly. Where you can be overweight and still look around the average or perfect weight.

You need to sleep as well. Try drinking the beer around camp, near Hosea's bedding, and eating big game meat.

I have never been able to get Arthur overweight. I'm just not good at feeding him.

Only .0000001 of players have seen more than the average because it is even more screwed up than looking for a new glitch to disable the cock-eater from New Austin

Underweight Arthur doesn't look underweight. He actually looks... normal. This is the problem. Another thing is, even when I was trying to make him fat he didn't get fat. xD I made him ate all of the meat and things he could eat and.. still not fat. Been trying this for some time. And another thing is Red Dead Online - my character is always overweight because why not. And I don't even try. Lol.

How do you maintain perfect, you need to eat regularly and run a lot?

It doesn't, there are plenty of fat ppl in rdr2. Arthur and john are outlaws , constantly getting into gunfights or escaping lawmen, they need to be healthy

Arthur needs to get on that Dwayne Johnson Diet: Get older, get bigger, workout, do nothing but eat all day, go to bed, get 10 hours, get back up and do it all over again. Also, don't forget the enhancement drugs.
