He'll get treatment, and eventually recover fully, Alfie won't be killed off, they've already done Lola, and her demise, so it's very unlikely they'd go down the same route with Alfie.

I think the person who dies is Phil and I don’t think it’s a coincidence that the flash forward episodes in was shown in February, which is the same month that he joined the show back in the 90s

As much as Alfie still loves and cares about Kat. She didn't deserve Phil cheating on her with Lola's mother Emma. Really Lola's mother. Lexi's grandmother. How could Phil do this to Kat who he loves very much and who he's about to marry. Alfie finds out about Phil cheating on Kat with Emma Lola's mum and decides to tell Kat unless Phil decides to come clean to Kat and tell Kat himself. Either way Kat will find out and she'll be so devastated and heartbroken by Phil's betrayal. And Alfie will be the one who will be there for Kat, Bert,Ernie and Tommy. Because of Phil's cheating betrayal of Kat and her and Alfie's children Tommy Moon, Bert and Ernie Moon. The most important people Alfie Moon cares so deeply for and about. And he always will.
I thought this was an old story not 2023, well that is EE for you the same recycled storylines

Phil's writing has been shit for years now you need to let go of the past yes he was an icon in the 90s and early 2000s but currently he doesn't add anything to the show all he does is give out cringe dialogue and empty threats.
