Adenomyosis Ultrasound Report Example | Uterus USG Scan Reporting | Gynecological Sonography Cases
Ultrasound Report:
Age: 34
Clinical History: Complaints of heavy menstrual bleeding, severe menstrual cramps, and pelvic pain.
Findings:
Uterus: The uterus is anteverted and measures 9.0 x 5.5 x 8.0 cm, which is mildly enlarged. There is evidence of heterogeneous myometrial echotexture. Multiple anechoic areas suggestive of myometrial cysts are noted. The endometrium appears thickened and is well defined, measuring 1.2 cm in thickness. No discrete uterine mass is identified.
Adenomyosis Indicators: The ultrasound findings are suggestive of adenomyosis, characterized by diffuse myometrial heterogeneity and the presence of small myometrial cysts. There is also evidence of the "venetian blind" shadowing artifact arising from the endometrial-myometrial junction.
Ovaries: Both ovaries are visualized and appear normal in size and morphology. No adnexal mass or significant free fluid is seen in the pelvis.
Impression: The ultrasound examination findings are suggestive of adenomyosis, given the heterogeneous myometrial texture, presence of myometrial cysts, and endometrial thickening. Clinical correlation is recommended, and further evaluation with MRI may be beneficial for more detailed characterization of the adenomyosis and to rule out other potential causes of the patient's symptoms.
Recommendations: Clinical follow-up with a gynecologist for management of symptoms and further evaluation.
Consideration of MRI for comprehensive assessment of adenomyosis extent and to exclude differential diagnoses.

    Thank you sir for this nice lecture. Do you have any case of how early adenomyosis appears? I have encountered cases, where there might be minimal striped shadows and thickened myometrium, but with no other finding and with no signs and symptoms. I end up just calling it a normal scan.

    No, the appearance of venetian blind pattern is not compulsory for diagnosis. Adenomyosis can occur without the presence of venetian blind pattern. It may present with myometrial cysts and heterogeneity.
