Abdominal Examination - Clinical Skills - Medical School Revision - Dr Gill
Given that we're now full vaccinated, and able to test in order to protect against the risks of COVID19, it seems appropriate that we return to some of the videos which were filmed during the lockdowns, specifically so we can complete every step of the examination.
Previously wearing masks meant it was not possible to safely film without the masks, thus we lost the steps looking at the patient's mouth. Hence the refilming with these complete examinations
In this video, we demonstrate how to perform a clinical examination of the ABDOMINAL SYSTEM for your medical school Clinical Skills OSCE. As the gastrointestinal exam is a core skill when it comes to examining patients, students should assume that an abdominal assessment is a high yield station for any clinical exams or clinical assessments.
For a passing grade in your Clinical Skills OSCE, for the Abdominal exam follow the approach of:
- Inspection
- Palpation
- Percussion
- Auscultation
HOWEVER, an abdominal examination OSCE station does not just involve pressing around the stomach, this video also demonstrates some of the specialised examination techniques required in examining patients such as shifting dullness for ascites, and checking the size of the liver which may be seen to be enlarged in cirrhosis as an example.
Diarrhoea, pain and general concerns about the abdomen are common reasons for patients to see a doctor, and in any speciality is probably the most commonly performed patient examination

Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
However during OSCE assessments. Different medical schools, nursing colleges and other health professional courses will have their own preferred approach to a clinical assessment - you should concentrate on THEIR marks schemes for your assessments.
The examination demonstrated here is derived from Macleods Clinical Examination - a recognised standard textbook for clinical skills.

Have x rays done. If it's gastrointestinal then unfortunately you're gonna need to get scoped also to make sure it's nothing concerning. Just go to hospital inpatients and they'll have to do the checks.

That’s why I have much better luck with alternative practitioners. They actually touch and examine you and try to reach the root of the problem. Traditional medical is great for diagnosing cancer or something extreme, but they’re not very great when it comes to modifying diet or lifestyle issues to prevent your problems from becoming worse.

That has nothing to do with alternative practitioners which can be dangerous. Like cars, doctors are of different qualities, but hopefully all to a minimum standard. Examinations are a adjunct, but not a main source of diagnosis. 80% of your diagnosis will comes from the history, the examination / investigations are to confirm/reject your differentials

And still doing the assessment in the wrong order. Auscultate, percuss, palpate. From stimulating bowel sounds to missing that aortic bruit and rupturing an aneurysm upon palpation. There's no reason not to auscultate first and several reasons not to palpate first.

This video reminds me of my childhood, when the doctor made all these procedures. Old-fashioned but effective way of taking care of the patient 's health. The voice of the doctor is so soft and calm. It helps relaxing

If doig a video on ADHD it would be good not to focus on ppl misusing the meds but on the genuine signs symptoms. Plus the risks without treatment (meds or specialist coaching/psychology).

Always loved this video but didn’t realize exactly why it was one of my favorites until now, lol, it’s because I’m so jealous of David’s flat stomach and chest 😂😂😂. How can I get a check up like this to ensure that my health is okay? I mean I feel well enough but with how thorough and attentive you are with your examinations it makes me wonder, how would I know if there is anything wrong if I’m not getting it checked out? Just like how you go to get a dental checkup I suppose as an example?

I wish you were my GP. I've recently had a bad accident and my local doctors don't seem very hands on. Trying my luck again at the end of the week while I go to follow up on an X ray.

Often it isn’t too hard to deal with. You can get the patient exhale as you are doing in pressure, and get them to focus on others things But everyone is different 😊

There are cardiac and abdominal causes it is important to keep in mind From an abdo perspective, one possibility is a problem with low protein But there can be many reasons

Often talking therapies are a good plan The NHS has several free apps that can help people with their own mental health But it would be worth while speaking to your Gp

No. There was a theory it might. But it has not been supported by the clinical skills research Auscultation is left to last, as it is the weakest sign clinically

Basically you DONT want to be on the receiving end. The only patient I’ve ever examined starting from his hair and finishing at his toes was VERY unwell, and we had no idea why Something had happened in an operation that we were not aware of. Took some very creative thinking to find it

There are different ways of doing examinations. This appears to be one of those differences Please check out MacLeods Clinical Examination as the reference text which is very commonly used at UK medical schools. I would highlight, that I can see a RATIONALE for flexion of the knee joint. However, I’d be interested to know the actual clinical implication of doing / not
