Well, Yako implies that it can be still the real Mitsuba, even after all these things he went through. Because the part of his original soul is still there and it contains his lost memories so... he only needs to remember stuff. The memories are hidden somewhere, they still exist inside of him.

One thing I have never herd mentioned about Mitsuba Sousuke is the fact that he no longer exists. Part of him does, but Mitsuba him self dose not. What I mean is Mitsuba Sousuke lived and died as a human, then he became a ghost and made a wish to Tsukasa. His price was his reasoning, or a piece of his soul being extracted form him. Ghost are made out of soul so basically it was a piece of him. In other words, Tsukasa took Mitsuba's reasoning or the reasoning portion of Mitsuba's soul as a price for his wish. After the roomer changing thing, Mitsuba Sousuke proceeded to disappear form existence forever. Tsukasa then put the reasoning portion of Mitsuba's soul (the piece he took as the price) into a new hand made body and named it Mitsuba. He then told Kou that this Mitsuba is an entirely different person and the Mitsuba who lived and died as a human no longer exists in this world. Basically: Tsukasa only put the piece of Mitsuba's soul that he took into the new body, and he only took the reasoning portion of Mitsuba's soul. Therefore, the new Mitsuba only has the original Mitsuba reasoning and nothing else.
