In the first incident, I can't help but wonder if the employee ignored any safety policies, or if there were any in place to begin with. Anyone who works around machinery of any kind, should be mindful of their environment as the timing of any movements can be unpredictable.

in the first incident really he just was probably trying to fix a problem with a bit of machinery but slipped and fell

OSHA requires prior to any preventative maintenance or equipment/machinery repair, that any equipment which could release any kind of energy in a hazardous way (electrical energy, hazardous liquids or gases, kinetic or mechanical energy from moving or falling equipment, etc.) be locked out of operation with a unique key and lock and tagged with a notice to any personnel that the equipment cannot be safely operated. If this is in the US, then yeah, there are definitely a lot of safety protocol and best practices which were ignored.

This animation is pretty impressive for beeing 13-14 years old
It feels wrong to watch these for entertainment purposes.
