    Sir, first of all nice video as always, sir sometimes while on giving closed dressing pt experienced pain may be because of pressure, bleeding, instrument crossing apex, chemical etc, so can we give only cotton after BMP?

    Yes, place cotton and temporary filling over it. Call after 3 days, then give calcium hydroxide. Regards👍🙏

    greetings to sir.. . it would be of great help if you could guide us about "how to diagnose and identify which case is vital, non vital, which case is necrotic or suppurative ?.. and also the line of management in these cases.. the diagnostic part has always been confusing since college days. hope to hearing from you.

    Experiments in a Nutshell: Vital tooth- No ICM Necrotic tooth-ICM but no antibiotics. No issues in both cases proceed further in the next sitting!

    Sir if we prepared all 3 canals in case of molar with good irrigation and the obturation was done and after 4 days pt complains pf swelling n pain and after xray the gp is at level of apex (mesial)in iopa so we have to do re-do the rct of only tht extedend gp canal or all 3 canals and also if without re rct the swelling subside in a week can we proceed to crown cementation?

    No need to repeat all the canals. Mesial canal can be reobturated if swelling or pain doesn't relieve with antiinflammatory drugs🙏🙏

    You can use calcium hydroxide for vital cases. Watch my video on intracanal medicaments. In this video I said so because it's an experiment to evaluate your BMP.

    @@nisa-vf6nk Because calcium hydroxide can dissolve pulp tissue and then you will not be able to evaluate your BMP. Remaining pulp tissue after BMP will pain if there is no medicament. In that way you will get to know that your BMP has removed all pulp tissue or not.🙏🙏

    Sir 20 no.k file if crossed beyond apex it will cause pain In this experiment we need to keep 20no.k file till working length This is what you are trying to teach,I think so

    Most important when patients say ...the other doc Saabs RCT is going well for so many years but you did and its painful...while in x-ray we clearly see that RCT is compromised....how to explain this to patients as well as to ourselves?

    Tell the patient, Its complicated case difficult to save but if you say so we can proceed but there can be some pain. In this way, if there is no pain after RCT, patient will be impressed and if it pains then patient will say doctor already said it can pain😄😄

    Thanks sir for the informative video...I have two queries sir...can we give an open dressing in necrotic case and I generally prescribe antibiotics in necrotic cases with swelling because patients are in a lot of pain...can these both things be done as you asked not to do them?

    Open dressing is a strict No in endodontics. Antibiotics can be prescribed in necrotic cases when systemic symptoms like swelling is expected or already there. Improve you BMP technique, make sure you are not pushing debris in periapex and complete BMP in one go.

    After single sitting pain can last 3-7 days but not always. Single or multiple sitting RCT have different indications. I will cover that in coming videos. 🙏🙏

    But sir in our practice we often see many compromised rcts going asymptomatic for many years...and sometimes after putting gud effort RCts fail...how to explain this ?

    Case Selection is the reason. The compromised RCTs you are seeing may be vital at initial stage. The dentist who duf that RCT select cases wisely and handle patient more precisely. Suppose if you remove all teeth which are necrotic, abscessed or remain symptomatic after BMP. You will left with simple vital cases. Regards

    Use of antibiotics will suppress the infection and you will think your RCT is going in right direction👍

    If the pulp is vital,and we access canal,than antibiotic is necessary if patient already had taken antibiotic for 3 days before RCT.
