180 - AMA #28 [sneak peek]: All things testosterone and testosterone replacement therapy

In this “Ask Me Anything” (AMA) episode, Peter and Bob discuss all things related to testosterone: what happens when testosterone levels are low, and the potential benefits and risks of testosterone replacement therapy (TRT). They explain the physiology of testosterone, how it works, and how its level changes over the course of a person's life. They have a detailed discussion about existing literature, which reveals vast potential structural, functional, and metabolic benefits of testosterone replacement therapy. They also take a very close look at potential risks of this therapy, with a focus on the controversial effects on cardiovascular disease and prostate cancer.
In this sneak peek, we discuss:
00:00 - Intro
00:10 - A primer on the hormone testosterone and how it influences gene expression
09:00 - How the body naturally regulates testosterone levels
In the full episode, we also discuss:
-The defining threshold for "low testosterone," how low T impacts men, and why free testosterone is the most important metric;
-When it makes sense to treat low testosterone;
-The structural and metabolic benefits of testosterone replacement therapy;
-Body composition changes with TRT;
-Changes in bone mineral density with TRT;
-The metabolic impact of TRT: glucose, insulin, triglycerides, and more;
-A study investigating testosterone replacement therapy for prevention or reversal of type 2 diabetes;
-The impact of TRT on metabolic parameters and body composition-A study comparing results from continuous vs. interrupted treatment;
-The controversy over TRT and cardiovascular disease;
-Two flawed studies that shaped perceptions of risks associated with TRT;
-The controversy over TRT and prostate cancer;
-Other potential risks with testosterone replacement therapy; and
-More

    People wanna watch your stuff but people aren't gonna subscribe just to see this video. Sometimes it may be worth releasing something that could be super appealing to a large audience where everyone can listen to the whole thing. That would bring more subscribers than teasing a little bit expecting people to sub for that. Hate to see such good content under viewed.

    Peter got one thing wrong here. The negative feedback to the hypothalamus comes from estradiol, not from testosterone. This is why men can use aromatase inhibitors and selective estrogen reuptake modulators to increase gnh/LH/testosterone levels.

  • @StrongManLiving

    I'm 34 year old man with 3 children. I would say I'm healthy however do have quite a bit of stubborn fat around mid section I can't get rid off. This year I have lost 3 stone just from intermittent fasting and feel much healthier. I recently had 2 sex hormone blood tests a month apart and both came back with very low testosterone, however I don't have the typical symptoms of low T. For example I have a very high sex drive/libido and have high quality sperm. The issue I'm having is no matter how hard I work in the gym I can't seem to gain much muscle or lose weight. I eat lots of meat, fish, liver, eggs, yogurt and a small amount of carbs. I take supplements including vitamin D, B vitamins, Boron, Zinc, Magnesium and some other supplement that supposedly support healthy T levels. So basically I'm doing everything I can to raise T but it's not working. What am I not doing right?

Vitamine ABCDEFish and Zink But my T didn't go more than 320 Finally my endocrinologist prescribed me Androgel 40.5 daily. I hope i go over 400 with Androgel

    You should try to get injections. You can do them once a week. Even with an insulin syringe... (Don't even feel it) and is far better than the gel. The gel has to be applied daily. It stinks. It's greasy, and not much gets absorbed through the skin, and it's not as easy to maintain a stable level with the gel. It's a lesser medication. But the Pharma companies make a killing on the gel v. Testosterone Depot.

    Please, please, please don't perpetuate the conventional wisdom whereby testosterone and TRT apply only to men. I heard your podcast with Drs. Bluming and Tavris and I bought their book shortly thereafter. If I'm not mistaken, testosterone is never mentioned in that tome. Thankfully, my doctor was not so rigid on the topic and a small dose has made ALL the difference in how I feel as a menopausal woman.

    @@jeffk.2736 I don't know the exact dosage, but women in their prime only make something like 10% of the amount that men make, so the dose in women's hormone replacement therapy (HRT) would be pretty small. My doctor recommended pellet therapy, which is inserted under skin for slow release for 3-4 months. The company that makes the pellet I use is called Biote, so you might look that up for more info on their approach (this is not a sales pitch for them; just one resource you could read up on). I have to pay out of pocket, but it's only about $100 per month, and worth it. Thanks for caring about your wife's symptoms! No two women are the same and her experimentation might take awhile, but don't give up.
