When treatment of heavy menstrual bleeding is the primary indication, best results are usually seen in the first 5 years. Kyleena (19.5mg) is still 5 years.     Im sterlised now ive had the iud and fell pregnant on it with my 3rd child. Im so glad that i have been sterlised yes the pain after surgery was bad but was controlled automatically. I feel like now im finally free of all the contraceptive hormones forever. None never agreed with me i had been taking contraceptives since i was 16 years old im now 34 years old was sick of tierd of all the side effects that i was having migraines, mood swings , weight gain etc. Thank god for the surgeon's that can perfrom tubal ligation because i no one thing i have no regrets

    I got kyleena snd I'm so glad I did. If I loose my health insurance I still have some coverage. But with recent events I'm thinking of having tubes tied to protect myself.

    Bah. My problem is that I love that my IUD stopped my periods, but I hate having a countdown in my head of when i need to get it replaced, and i hate having it inserted. I haven't had it removed yet but i dont want to keep getting it inserted and removed every 7 years. I like the permanence I could feel from the tubal ligation - but i don't want to have a period again or risk surgery. Blah!

    “I hear you”! I’ve had many patients continue with a new hormonal IUD, even when their partner has subsequently had a vasectomy- just because they love the effect it has on their periods (similarly, I’ve seen lots of women who have already had a tubal ligation, who later end up with the hormonal IUD to manage new menstrual issues). I know an IUD insertion is not “fun”, but at least the effects are long lasting! Good luck with your decision! -Dr. Dervaitis

    This is a very very different procedure of tubal ligation than i assisted with during my clinicals. The dr entered the patient vaginally and with a scope cauterized the tubes through the uterine canal.

    If my wife is going for a C-section, 36 years, second baby due, and no plans for future pregnancy, is Ligation better option than IUD. We would not need any separate surgery, because she is already going to have a C-section.

    Hello- You are correct- there is no significant added risk when a tubal ligation is performed at c-section. In this circumstance, the biggest “risk” to consider is risk of regret (the procedure is irreversible, and so the patient needs to be absolutely, 100% sure that she would not, in any circumstance, want to be pregnant again). I’d recommend that your wife discuss this further with her doctor. -Dr. Dervaitis

    Thanks for your question! I have never read anything in the Gynecology literature/evidence regarding copper toxicity due to copper IUD use.     No weight gain associated with tubal ligation. No impact on hormones or the menstrual cycle (it is simply a physical separation/ blockage of the tubes... no impact on ovulation or cyclic hormone fluctuations). -Dr. Dervaitis

    I’m not aware of any studies that support the concept of a “post tubal ligation syndrome”. Tying the tubes does not effect hormones or the menstrual cycle (although stopping the birth control pill, implant, hormonal iud, or any other hormonal contraception, would potentially change the cycle when discontinued after tubal). Discussion of the complications of Essure are beyond the scope of this channel. If you have further questions about permanent contraception, talk to your doctor. -Dr. Dervaitis

    Hello Dr. Kristina! - For context, I'm also in Canada. My doctor has explained that I can have a "tubal ligation" where my tubes would be entirely removed after my C-section. (I'm currently 7 months pregnant). My major concern is that this procedure may affect my hormones going forward. - I've read and heard a lot about Post Tubal Ligation Syndrome (PTLS), where women's hormones are in fact impacted after a tubal ligation. Maintaining my hormone levels is critical to me, because I have Osteoporosis, and NEED to maintain my hormone levels so not to worsen my bone's condition. My doctor said that there should be no impact to my hormone levels because removing the tubes doesn't impact blood flow to the ovaries, etc. ... however I keep reading about women who have PTLS, and doctors who verify that this is a possible side-effect. I'm wondering if the disconnect is because Tubal Removal/Tubal Excision is a different procedure than what's normally referred to as "tubal ligation" (blocking tubes, clamping, etc) / maybe in the United States, "Tubal ligation" normally means cauterization or something?... OR maybe PTLS is something caused by surgical error, etc? So I guess my question is - will I be able to maintain my hormone levels after when specifically having a full Tubal Excision after a C-Section? Is blood flow to the ovaries or any other part of the female organs affected by full tubal removal? Do the tubes serve any purpose in regulating hormones? (I heard another doctor say that some postulate that the tubes have special signal cells in them that signal the brain to product certain hormones, although I can't find evidence of this). Alternatively, would an IUD negatively impact my progesterone levels, or disrupt my hormones overall?

    The Fallopian Tubes do not have any function in regulating hormones. Please see video #10 for info on hormonal side effects of the IUD (overall very low, very little of the progesterone gets into the blood stream). -Dr. Dervaitis

    Hey! I am about to go in for a hernia surgery/replied is it okay to get it done with the tubal ligation? I have 4 children and can’t get my husband to go in for vecetemy. We been talking about it for about 4yrs and it’s not happening. 🤷🏽‍♀️ any advice is appreciated thanks 🙏🏽

    You will need to discuss this with your doctors, as it might require coordination between two different surgeons to do both procedures during the same operating room time (but it is theoretically possible). -Dr. Dervaitis

    I am in dilemma. Been on the pill for 18 years now and want to quit it to see how I am without the synthetical hormones. I am 33 years old married and we are happily childfree. Because I don't know how my natural cycles are, i really want to first experience them, and seeing if quitting the pill does anything for me in the positive way, before I can make the right decision on my next type of birthcontrol... the fact is that I am very afraid of getting pregnant while just using a condom and to be honest sex is not the same with a condom... ppfff hate to be a woman at this moment 😑 if anyone has some advice, please share!

    Kurbie- “Natural” cycles mean preparing the uterus to carry a pregnancy. To be “natural” means accepting a (very high) chance of pregnancy with each cycle. Good luck with deciding on your future contraception, but remember that contraception is all about preventing what Mother “Nature” wants (and what you don’t...) - a pregnancy!     I've been using birth control pills for several years. In the last couple of years I was experiencing bad migraines every day and it would not let me function. I stopped using them I am now 21 weeks pregnant I have a c-section scheduled and im thinking about tube ligation but I'm nervous about it.     I am 21 with 2 kids already a boy and a girl AND I don't want any more kids 😭😩 i love my kids and I can't afford any more kids I have the copper IUD but recently I had to go to my OB because I had pain when holding my pee so turns out my IUD was low I'm really scared to get pregnant again !     IAM 32 with 3 children and don't want any more children. Although I love my babies I didn't really want anymore after my 2nd other than to have a son... Now I have my son. Birth almost killed us both and I will say I have never been so scared I remember begging God not to wake me up if my son didn't make it. Thank God we both made it out fine thanks to our wounderful Dr, God, and nurses. My question is my sister went through menopause early and from what I've read chances are I will too should I just wait and hope for menopause before another child or go ahead and have the surgery to be sure that there are no more children in this already big enough family. Also how far into menopause do you have to be before you are no longer fertile

    No. It has absolutely zero impact on hormones or ovulation. It literally only blocks the tubes (no hormonal mechanism of action).

    A tubal ligation usually takes less than an hour. If uncomplicated, most patients return home the same day. Recovery for laparoscopic procedures is usually quoted at around 2 weeks (although there are not usually any activity restrictions post operatively, it just depends on the amount of discomfort and how that might limit activity).     Hi there! I have a history of migraines when using hormonal birth control methods and have consequently used Paragard as my primary birth control method for almost 20 years. However, I’ve now been diagnosed with anemia and the Paragard is exacerbating it. I’m hesitant to try Mirena or Kyleena because of my migraine history; are these hormonal IUDs associated with migraines, weight gain, or skin reactions like acne?

    Hi- The levonorgestrel IUD is: -NOT proven to cause weight gain -NOT contraindicated in migraine sufferers: it’s estrogen, not progesterone, that we need to avoid in migraine patients (there’s no estrogen in the IUD!) -may cause acne in a small percentage of women. Watch video #10 for more info re: possible hormonal side effects. -Dr. Dervaitis

    i tried all Hormonal birth controls. I always had serious side effects and could not continue. I got a hormonal IUD, i didn’t have any side effects like the ring patch and various oral birth control.

    I'm 46 years old. I had my tubes cut tied and burn when I was 25 years old. Is it possible years later the surgery could fail I could become pregnant ??? Thank you so much for making this video

    Although fertility declines significantly in the late forties, pregnancy is certainly possible. With tubal ligation, the chance of pregnancy is less than 1%. That said, declining fertility plus tubal ligation at age 46 would make pregnancy very unlikely (but still theoretically possible).

    I had a hernia repair surgery back in 2012. I have had the copper IUD for about 2years. I want it removed due to the fact, i now get frequent yeast infections right before my cycle.     Thank you for all the great videos! In regards to the copper IUD, is there any copper released into the blood stream? I’ve read some stories of women who have had “Copper Toxicity” from the IUD. I’m sure it’s rare if it does indeed happen, but what are your thoughts on it?

    I know this post is old, but I couldn't deal with the cramps and heavy bleeding from the copper one. I had it for a year and it was terrible. I switched to kyleena two years ago and ita night and day.

    No, and no. Removal of the tubes (or cauterizing or clamping them) has no effect on hormone production or function. For this reason, it also has no effect on menstrual bleeding. -Dr. Dervaitis

    Yes. The failure rate of each is very low, most women choose one option or the other (but both methods combined is certainly possible).

    I have had tubal ligation and also have a copper iud in place also. So yes it's OK to do this and also it gives the extra piece of mind.

    How could I have a solid erection the morning of my visectomy but from that surgery I could not get an erection for a year before I went and had an penile implant.The tube in my penis bulges out the side of my penis.It hurts when not erect but it still has pain in my penis since this implant was put in me 4 months ago almost.How could that happen?

    I am so sorry to hear this. Regret from tubal ligation is one of the big risks, which is one of the advantages of an IUD (since the IUD is equally effective but easily reversible). I don’t perform tubal reversal procedures so can’t advise you. Some patients choose to do fertility techniques like IVF (to get around the blocked tubes by directly harvesting the egg and fertilizing it with sperm in the lab). You will need to talk to a doctor in your own country for advice. All the best, Dr. Dervaitis

    Hmm. What kind of people would you say have been the best fit? What kind of medical history? Like for myself I have a high drug tolerance have known that since I was 9 🤷🏻‍♀️ and even giving birth I could walk on a epidural shot. Lol but its just been sad for me, and I feel like there's no real family life anymore. I'm getting deeper into debt too that started from my ex And I have also been su¡c¡dal and annoyed. That stuff. And I have another question but I'd like to see if you see this first bcz it's kinda embarrassing, but depending on the answer I might share .... .. Like idk, I have fairly strong genes but I also think I might be diabeticish too noow since having my son. Ugh I get siiiiick sometimes if it's not enough it's annoying. It's tolerable rn but annoying 🙃 I could prob tolerate it for a while I've only been occasionally unhealthy even doing drugs for three years 🤷🏻‍♀️ and once before that when I was 18

    They mistaken back herpes I got when I was 4/5 months pregnant having a bath at the homeless shelter (3 ppl a room) told me to take a Epsom salt bath Ya ok fine I knew that wasn't the answer tho I knew it was herpes I've had herpes for 14 years. Genital and it's not nearly as bad as my back But no, idk what I'm talking about Take a Epsom salt bath it is then I later convince them. Yo I got herpes and I'm sexually active. I don't break out much (and I was briefly but I didn't need them) one day my back was doing its thing so I took a herpes medication and it was gone 50 minutes. I dont feel safe lmao but thankfully they don't do the crazy procedures in this town. I'd rather go to the city and get a female to but I'm also not gonna get good advice here Like I THINK I'm good but I've also never had any sort of surgery before either

    This vid is leaning super heavy on the IUD method. I've had both kinds of IUDs. First one stabbed my partner in the dick. 2nd one gave me nearly constant BV and Yeast infections (cleared up after it was removed). They can become dislodged which means pregnancy. I am now firmly in favour of Tubal ligation as a permanent method of birth control, if the tube is severed and cauterized not even God could make a pregnancy. Cheers.

    The failure rate of tubal sterilization (by any method) is 0.5-1% (it’s possible over time for a communication to develop between the tube and uterus, whether through or near the scar tissue). The only method with a 0% chance of pregnancy is abstinence. -Dr. Dervaitis
